Sterling Heart Care is an independent cardiovascular treatment center located on Maple Rd. in Amherst, NY. We work closely with area Cardiology and Primary Care health teams to ensure patients have access to a full scope of cardiovascular treatments and services.

How do I learn more about Sterling Heart Care and its services?


If you or a member of your team have specific questions about our services or you would like to schedule a lunch and learn at your office, please contact our office and we will be happy to coordinate with you.

How do I refer a patient to Sterling Heart Care?


We have an online, HIPAA secure, referral form option. Patients can also be referred by faxing a referral form to our office. Our fax number is (866) 778-5747. If you need referral forms, please contact our office and we can have them faxed or delivered.

Do you have informational material that I can pass along to my patients?


Yes! We have brochures and informational materials for both our cardiac rehab and ECP services. We also have informational material that is geared towards clinical professionals such as Physicians, Nurse Practitioners, Physician Assistants, Registered Nurses, Exercise Physiologists, and Registered Dietitians.

Will I be kept informed of my patient’s progress at Sterling Heart Care?


Absolutely, we send out periodic reports and updates to referring physicians and their team’s. These reports will include any updates to your patient’s care plan as well as a report on their progress.

Will Sterling Heart Care make any treatment decisions unrelated to the services my patient was referred for?


Your patient is your patient. At Sterling Heart Care, we work to support our referring physicians in the care of their patients. We do not make any treatment decisions unrelated to the services a patient was referred for unless we receive prior approval form their referring physician. We often find ourselves communicating with referring physicians on medication changes. As patients participate in our cardiac rehab and ECP programs, we often find that patients will need reductions in blood pressure and anti-anginal medications as their health status improves.

Is it possible to tour the Sterling Heart Care facilities?


Yes it is, we often have referring physicians and their teams requesting to visit our facilities. If you’d like to schedule a time for a tour and discussion, please contact our office and we’ll be happy to accommodate you.
